The present report commenced being drafted at 22:05 hours of 03/01/2018, at which time
we arrived at the venue of the accident and was completed following the inspection which
was made in turn on persons and things at the office at 03:35 of hours of 04/01/2018.
There are annexed a chart of the venue of accident at 1:200 scale and 0 photographs.

(3) To be completed is the type of accident, e.g. frontal collision or south - front impact or
under - angle impact or side - frontal impact or reversal, diversion or run - over of a
pedestrian etc.
(4) To be completed are the particulars which are requested and furthermore in the event
of trucks there is stated the gross weight and the net weight and the tare weight. For
bicycles there is stated the cc of the motor, for buses the number of seats, etc.
(5) To be completed are the following: the full name, even name, father’s name, mother’s
name, the year and place of birth, the address of residence (area, street, number), the
driving license number, category and date of each issue as well as the agency which issued
it.
(6) To be completed are the full identification particulars as above (par. 5) with the
addition whether there were drivers or passengers and of which vehicle, or there were
pedestrians (in the passenger there is noted also the position).
(7) To be completed are the full identification particulars as above (par. 5) and there is
explained where exactly each of them found themselves at the time of the accident (e.g.

co - passenger of the 2 car, on the pedestrian way of the street … etc.). Also, mention is
made whether the witnesses were found at the place of the accident by the drafter of the
present report, or whether the particulars were given by another police officer whose
arrival preceded, in which case there is attached a stub of a noting pad.

(9) To be completed are the particulars requested. In particular for the visibility, it is
clarified whether it was restricted at the time of the transition of the Inquisitorial Official
to the place of the accident.

(13) To be completed in detail both are the collided surfaces of the vehicles and the
material damage thereof.
(14) To be completed is the course of the vehicles and the pedestrians on the basis of the
found exhibits or testimonial depositions and there is noted if it is normal or not.
